Evidence-based physical activity guidelines for people with spinal cord injuryAudrey L. Hicks and Kathleen Martin Ginis, Department of Kinesiology, McMaster University, Hamilton, ON, Canada, Spero Ginis, SCI Action Canada There are more than 85,000 Canadians currently living with a spinal cord injury (SCI), and an estimated 4,300 new cases each year. While the amount of disability associated with an SCI is unique to each injury -- depending on both the level and completeness of the damage to the spinal cord -- physical deconditioning is a common consequence.  This physical deconditioning can further exacerbate the impact of the SCI, and is associated with increased risk for chronic secondary health complications. It has been suggested that much of the excessive (and early) morbidity and mortality in people with chronic SCI is caused by inactivity-related illnesses, such as cardiovascular disease, type II diabetes and osteoporosis.  In addition, the loss of fitness and independence associated with physical inactivity significantly impacts quality of life and community participation.  These facts highlight the importance of promoting physical activity to improve health, fitness and overall quality of life within the SCI population.

Over the past 20 years there has been consistent growth in the SCI research literature related to the benefits of physical activity for increasing fitness, improving psychological well-being and decreasing risk factors for chronic disease.  At the same time, it has become increasingly apparent that accessible physical activity programs and information about how activity can promote health are two of the services most desired but least available to people with SCI living in the community.  Such information is available to healthy Canadians in the form of physical activity guidelines, however these guidelines were based on evidence from the able-bodied population, and thus are not necessarily appropriate for people with SCI.  Until now, there have been no evidence-based physical activity guidelines to inform exercise prescription for people with SCI.

Through funding from the Rick Hansen Institute, we have gone through a rigorous scientific process to develop the first evidence-based physical activity guidelines for people with SCI. The guidelines were developed by an expert panel from across Canada, led by researchers at McMaster University, in partnership with SCI Action Canada.  We followed the internationally recognized Appraisal of Guidelines, Research and Evaluation (AGREE II) protocol, ensuring that the guidelines were unbiased, comprehensive and of the highest scientific standard. The official cross-Canada launch of the SCI Physical Activity Guidelines was on March 17, 2011.
The Guidelines:
For important fitness benefits, adults with a spinal cord injury should engage in:
at least 20 minutes of moderate to vigorous intensity aerobic activity 2 times per week,
AND
strength training exercises 2 times per week, consisting of 3 sets of 8-10 repetitions of each exercise for each major muscle group.
These guidelines represent the first ever, evidence-based physical activity guidelines to be developed specifically for adults with SCI.  They are designed in a way that they can assist clinicians and exercise programmers in prescribing and promoting physical activity and exercise as well as, act as a practical guide for adults with SCI who want more information on the types and amounts of physical activity required for fitness benefits.
